Capability to report on assigned Qualifications History

Create a report that would provide the History of Qualifications assigned to an Employee during a period of time. 

The Report would be helpful to address the need for station leadership to:
1) Retrieve and report on who made the Qualification Assignment and when; i.e. - Who made the assignment and date stamp

2) Search within a date range; i.e. - a Begin and End date range 

This report would helpful to mitigate shift and bidding errors that can be cumbersome to resolve and assist leadership to identify for coaching individuals who make the incorrect Qualification assignments.

This is a good idea in order to prevent accidental scheduling for a task not signed off on.
