Show phone numbers in the survey results for Notification-4 and Notification-10 Responses

When looking at the survey results for Notification-4 or Notifcation-10 Response you see the Name, Location, Class, Response, and Source.  There is no way to see any contact information for the people that were called/emailed.  If I want to contact someone that chose a response to "report for duty"(Choice 1), I have to look up their contact information, either on the employee record or on the roster used for the Notification Response.  If the survey results showed contact information it would make it much easier to export and forward to those that would contact the person to "report for duty".
